HealthJay is a care engagement app that uses a Building-Block like interface offering person-centered access to healthcare recipients. Clinical care, chronic care, and social determinants of health are all delivered including walkie-talkie video chat with care providers. Users can manage care events in this ultra simple-to-use app using a whole-health approach. This gateway app takes care of recipients and care providers through care journeys with several transformative digital breakthroughs: • All communications are automatically translated into the users selected language with both text and high quality speech synthesis
